Complete Your Intake Online (Recommended)
Most intake forms should be completed through our secure patient portal at least 48 hours (2 business days) before your appointment.
- 1.Log in to the patient portal using your email address
- 2.Select your upcoming appointment and click "Check In"
- 3.Complete intake forms, emergency contacts, and consent forms
Alternative: Email Submission
If you need to submit additional forms not available in the portal:
- 1Download and complete the required forms
- 2Email to office@allenpsychiatry.com
- 3Include copies of insurance card (front & back) and photo ID
